Gr. 1 - 5 Registration Forms

When registering your child for school, you will need to provide proof of age documentation for your child (birth certificate, baptismal certificate, passport, Manitoba Health Card), as well as two pieces of ID for proof of residency (driver's license, Manitoba Health Card, tenancy agreement, offer to purchase documents, utility bill).
